We review standard hardcopy medical records and digital records, as well as depositions and sub rosa video footage. 

We have developed standardized formatting styles for our reviews, based on years of input from our clients.  The standard formats capture all relevant information, and may be further tailored to suit your needs.  Our standard formats typically include all major reports, including:

  • Initial Evaluations / Consultations / Doctor's First Report of Occupational Injury or Illness  - Including all major elements of these reports.
  • Final Reports / P&S / MMI / AME / QME / IME - Including all major elements of these reports.
  • All office visit reports, including Progress Reports / Interim Evaluations / Supplemental Reports / SOAP notes - Including all major elements of these reports.
  • Diagnostic Studies - Including MRI, CT, x-ray, ultrasound, EMG/NCV.  We can also address more specialized reports by request (pulmonary function testing, etc.).
  • Operative/Procedure Reports - Including major operative reports as well as procedure reports such as injections and ESWT.
  • Record Reviews - Including what was reviewed, and any comment by the reviewing physician.
  • Depositions - Our standard formats include a detailed summary of the entire deposition (excluding opening admonitions, etc.); however, this can be scaled down to your preference.   Some clients prefer only a mention of the deposition; others prefer that we review the depositions fully but note only those issues that are immediately relevant to their specialty.
  • Claim Forms - For our workers' compensation clients, including a brief overview of the most pertinent data (date and mechanism of injury, employer, affected body parts); we review all claim forms and amended claims, to provide an understanding of the scope and progression of the case.
  • We also make note of physical therapy and chiropractic treatment records, hospitalization records, personnel records,  and general medical treatment records, noting the facility and specific treatment, where applicable.   We can include lab reports, home healthcare reports, vocational rehabilitation reports, and anything else your office requires. 

We strive to provide reviews that are not only detailed, but succinct – to highlight salient details without dwelling on minutiae or items outside your specialty.  We save you time and money by giving you only what you need.  You won’t find descriptions of duplicates, lab work, insurance forms, correspondence, or other documents that aren’t immediately germane to the case.

Our standard formats are easy to read and constructed to mirror the content of a typical medical report.  We highlight patient complaints, diagnosis, and treatment plan in every report where applicable, as well as addressing history, prior history, contributing factors, and work status, if necessary.  For our workers' compensation clients, we additionally address impairment rating and factors of disability, causation and apportionment, vocational rehabilitation, restrictions, and future treatment plan. 

We securely archive all of our reviews, to be referenced in the event of supplemental  reports and/or future appointments.  We track the precise time taken for each review, including preparation, review time, and proofing and editing, for your reporting and records.
